The back cover identifies Island Records with Atco/Atlantic distribution and notes Printed in U.S.A.; it also credits Adrian Belew as producer for Macaque.
The jacket artwork has a quietly surreal pastoral wit: Belew, in a bright red suit with guitar, stands in a meadow before a rhinoceros while a white bird balances on the animal’s back beneath a wide, clouded sky. The artist and title appear in thin red cursive across the upper front, and the sealed shrink creates glossy diagonal ripples across both panels, with the corners and edges still wrapped.
The rear panel carries the musician and recording details, crediting Belew with guitars, effects, drums, percussives, and lead vocals, alongside Christy Bley, William Janssen, J. Clifton Mayhugh, and Audie Belew.
